Psychosocial Recovery Coaching
Supports for ME provides Psychosocial Recovery Coaching to assist people with psychosocial disability to live a purposeful and meaningful life of their choosing. We take the time to understand your individual needs, strengths, and goals, and support you to identify the services and supports that are right for you. Working collaboratively with your informal supports, service providers, and key stakeholders, we help coordinate and navigate the NDIS and broader mental health systems. Our recovery coaches support you to build personal capacity and recovery skills, including motivation, resilience, confidence, decision-making, and self-advocacy. We assist you to develop a personalised recovery plan that is practical, meaningful, and aligned with what matters most to you. We maintain ongoing engagement and are responsive to your needs, particularly during periods when increased support is required, supporting greater independence, social participation, and economic participation over time.
Support Coordination
Support coordination helps you to make the best use of your supports in plan. Support coordination is a capacity building support which helps you to:
- Understand and use you NDIS plan to pursue your goals
- Connect you with NDIS providers, community, mainstream and other government services
- Build your confidence and skills to use and coordinate your supports.
Specialist Support Coordination
Specialist support coordination is a higher level of support. The focus is on reducing complexity in the participant’s support environment and helping the participant overcome immediate and/or significant barriers in plan implementation. Specialist support coordination helps participants: address complex barriers that affect their access to supports & design a service plan for their support needs, where appropriate.
The primary goals of Specialist Support Coordination include:
